The majority of these deaths occur in sub-Saharan Africa.1 Since they are largely preventable, they represent a tragedy playing out every day across the continent. Progress on maternal health there is hampered by health systems that are understaffed, underfunded, and overwhelmed and thus too fragile and fragmented to deliver the required level or quality of care. Consequently, many countries in sub-Saharan Africa will struggle to meet the United Nation Millennium Development Goals for reducing child and maternal mortality by 2015.2
Nonetheless, some countries are making headway. Our recent work in Namibia, for example, suggests that coordinated, targeted interventions led by local stakeholders can accelerate improvements in maternal-health outcomes. The key is to work with local health leaders to develop solutions that improve the quality of health care, increase access to it, and promote its early uptake.
The resulting interventions being pursued in Namibia are straightforward and practical improvements in the training of midwives, cheaper antenatal clinics inspired by the design of shipping containers, operational fixes to reduce ambulance response times and wait times at clinics, a radio talk show to educate patients and stimulate demand—yet are collectively powerful. A closer look efforts offers lessons for other countries seeking to improve maternal health, as well as for health programs tackling HIV/AIDS, malaria, tuberculosis, HCV+ or many other conditions.

Patients with acute hepatitis C virus (HCV) infection appear to have an excellent chance of responding to 6 months of standard therapy with interferon (IFN). Because spontaneous resolution is common, no definitive timing of therapy initiation can be recommended; however, waiting 2-4 months after the onset of illness seems reasonable.

Treatment of chronic HCV infection has 2 goals. The first is to achieve sustained eradication of HCV (ie, sustained virologic response [SVR]), which is defined as the persistent absence of HCV RNA in serum 6 months or more after completing antiviral treatment. The second goal is to prevent progression to cirrhosis, hepatocellular carcinoma (HCC), and decompensated liver disease requiring liver transplantation.

Antiviral therapy for chronic hepatitis C should be determined on a case-by-case basis. However, treatment is widely recommended for patients with elevated serum alanine aminotransferase (ALT) levels who meet the following criteria[4] :

Age greater than 18 years
Positive HCV antibody and serum HCV RNA test results
Compensated liver disease (eg, no hepatic encephalopathy or ascites)
Acceptable hematologic and biochemical indices (hemoglobin at least 13 g/dL for men and 12 g/dL for women; neutrophil count >1500/mm3, serum creatinine < 1.5 mg/dL)
Willingness to be treated and to adhere to treatment requirements

A further criterion is liver biopsy findings consistent with a diagnosis of chronic hepatitis. However, a pretreatment liver biopsy is not mandatory. It may be helpful in certain situations, such as in patients with normal transaminase levels, particularly those with a history of alcohol dependence, in whom little correlation may exist between liver enzyme levels and histologic findings.

Patients with normal liver enzyme levels and minimal histologic damage noted on liver biopsy may elect to defer treatment until more effective and less toxic medications become available, whereas patients with more advanced liver injury may prefer to initiate treatment sooner. Patients should be informed that the treatment of HCV infection in the setting of normal liver enzyme levels remains controversial.

The virus can infect the person and may cause severe inflammation of the liver leading to severe liver damage and long term complications.
The infection usually begins in a gradual manner with vague symptoms like tiredness, loss of appetite, abdominal discomfort, nausea and vomiting, fever etc.

Slowly it may progress to jaundice in about 25% of patients. Of those infected nearly 40% recover fully and the rest become chronic carriers of the infection.

Of these nearly a third develops liver cirrhosis and of those with cirrhosis, up to 20% develop liver cancer.

New blood test could accurately predict heart attack risk-

According to The Heart Foundation, more than 920,000 Americans will suffer a heart attack this year, and many of these will occur without warning. But researchers from The Scripps Research Institute in California say they have created a blood test which may be able to predict whether patients are at high risk of heart attack.

The research team, led by Prof. Pete Kuhn, says that at present there is no test available that can predict the occurrence of a heart attack with good accuracy.

But they say their novel test, details of which have been recently published in the journal Physical Biology, has so far proved successful in identifying which patients are undergoing treatment for a recent heart attack and which patients are healthy. Test identifies endothelial cells in blood

The new test uses a "fluid biopsy" technique. It works by identifying the presence of endothelial cells - which line the artery walls - in the bloodstream.

According to the researchers, endothelial cells that circulate in the bloodstream have been associated with ongoing heart attacks.

Researchers say the HD-CEC test can accurately detect circulating endothelial cells in patients, meaning the test could be used to predict heart attack risk.

They believe that endothelial cells enter the bloodstream as a result of diseased plaque building up, rupturing and ulcerating in the arteries, which triggers inflammation.

They add that this damage to the arteries can lead to the formation of blood clots. This stops the blood flowing through the arteries, which in turn can cause a heart attack.

Using a newly-created procedure called the High-Definition Circulating Endothelial Cell (HD-CEC) assay, the researchers were able to identify and differentiate endothelial cells in blood samples of 79 patients, all of whom had already suffered a heart attack when their samples were taken.

The researchers also used the HD-CEC assay on two groups of patients as a control measure. One group was made up of seven patients who were receiving treatment for cardiovascular disease, while the other group consisted of 25 healthy patients. Blood test 'successfully identified heart attack patients'

The research team found that the HD-CEC assay was able to detect circulating endothelial cells in the blood of the patients through the cells' "morphological features and their reactions with specific antibodies."

Patients who suffered a heart attack had much higher levels of circulating endothelial cells in their blood, compared with healthy patients. And the researchers note that the cells were identified with "high sensitivity and high specificity."

To further confirm the accuracy of the HD-CEC assay, the researchers compared it with CellSearch - a test that has been approved by the US Food and Drug Administration (FDA) to identify circulating tumor cells in patients with cancer.

From this, the investigators found that the HD-CEC test was able to detect circulating endothelial cells more accurately than the CellSearch test.

The researchers say this is because the HD-CEC test "used a direct analysis method and was free of bias from an enrichment stage."

"Our assay effectively analyzes millions of cells, which is more work but guarantees that you are analyzing all of the potential cells," says Prof. Kuhn.

The investigators believe the technique is now ready to be tested on patients who show symptoms of increased risk of heart attack, but have not yet suffered one.
